[ceph-utility] Remove the ceph-mgr-cephadm package

Removes the ceph-mgr-cephadm package from the ceph-utility image.
This package runs systctl, which isn't always available in
downstream build environments. Cephadm isn't used in openstack-helm
and this package shouldn't be needed.
